Osteoarthritis of the hip is a common cause of chronic pain in the hip. This can affect people of all ages. The frequency of osteoarthritis increases with age, but not everyone experience chronic hip pain. Hip osteoarthritis is typically caused by wear and tear related to aging and worsens over time. The breakdown of cartilage leads to pain and inflammation.

About 90% of patients above the age of 65 years who have chronic hip pain also have osteoarthritis of the hip joint.

Osteoarthritis of the hip usually give the following symptoms:

  • Gradual and increasing stiffness and pain in and around the hip. Groin pain and pain in the front and side of the thigh. Some patients experience pain in the knee when having osteoarthritis of the hip joint.
  • The pain comes especially when you start walking. After a few minutes, the pain subsides, but then gets worse again after some time as you walk.
  • In severe cases, the pain may be present at night and cause sleep disturbance.
  • Stiffness of the hip joint is common.
  • Osteoarthritis of the hip causes pain when walking, reduced walking distance, pain when climbing stairs and some patients might even need a walking stick.

What is osteoarthritis of the hip?

The hip joint is comprised of two bones: the thighbone and the pelvis. The ball of the hip joint is made by the femoral head while the socket is formed by the acetabulum. In the healthy hip joint, the hip socket and femoral head are covered with a smooth surface of cartilage, ensuring natural and frictionless movements of the joint.

In the osteoarthritic hip joint, parts of the cartilage are damaged causing inflammation in the joint. This can cause chronic pain and reduced mobility of the hip joint.
Osteoarthritis is a lack of cartilage and the inflammation causes chronic pain.

What causes osteoarthritis of the hip?

In osteoarthritis, parts of the cartilage in the joint are destroyed. There may be various reasons.

The following conditions increase the risk of getting osteoarthritis of the hip joint:

  • High age
  • Overweight
  • Heavy manual work
  • Work requiring prolonged standing
  • Arthritis
  • Running and marathon training
  • Joint injury related to sports
  • Old hip fracture (either in the hip socket or femur)
  • Innate joint damage (e.g. hip dysplasia and Calvé-Legg-Perthes disease)
  • Infection of the hip joint

In some cases, hip pain is not caused by osteoarthritis of the hip, but osteoarthritis of the lower back. This is called referred pain and occur in about 10% of patients above the age of 65 years with chronic hip pain.

How is hip arthritis diagnosed?

Osteoarthritis of the hip is usually diagnosed on the basis of the medical history and physical examination.

We might use the following diagnostic tools to determine if you have hip arthritis:

  1. A description of your symptoms and medical history.
  2. Localisation and character of the pain
  3. Findings from the clinical examination

An X-ray of the hip joint is used to confirm the diagnosis.

What treatments are available for hip arthritis?

Treatment of hip osteoarthritis and hip pain aims to relieve the pain and reduce symptoms.
If there are restrictions of movement in the hip, the aim can be to reduce stiffness of the hip with appropriate treatment.

Which treatment to go for should depend on the degree of pain and how much your daily life is affected by the disease:

As a rule of thumb, we always recommend treatment for chronic pain-relief with gold implants before hip surgery, as gold bead implantation of the hip is an effecient and much more gentle and safe procedure without the need for prolonged rehabilitation in contrary to hip replacement surgery.

Thus, gold bead implantation is a good alternative to a total hip replacement.

Treatment of moderate to severe hip pain due to osteoarthritis?

Most of the treatments that may relief mild to moderate pain of the hip, might also have some pain-relieving effect in severe hip pain.

Below is listed the treatments which might relief moderate to severe hip pain:

Gold implantation (implantation of gold beads around the hip joint) is recommended for both mild and severe pain in osteoarthritis. This treatment is an good alternative to total hip replacement surgery.

Most of your patients experience either significant pain relief or no pain at all after gold implantation of the hip. The effect might be permanent in most patients. The treatment is performed under local anaesthetic, does not require hospitalisation and has no risk of complications.
There is no need for rehabilitation after a gold implantation of the hip.

Hip surgery may be recommended for severe osteoarthritis pain where other treatments are not effective. Unfortunately, a total hip replacement is no guarantee of pain relief. Few patients might even experience a worsening of chronic hip pain after hip surgery.

Like gold bead implantation, hip surgery might provide long-term pain relief for several years.

Total hip replacement comes with risks of complications and a need for months of rehabilitation after surgery. Therefore, hip surgery should always be considered as a last resort (read more about a total hip replacement surgery for chronic pain).

How to avoid aggravation of the pain?

In severe hip pain, physiotherapy provides limited pain relief. However, it is important that you use your hip on a daily basis to avoid worsening of pain and loss of function.

The pain might even get worse over time when you try to avoid using the muscles around the hip joint. Therefore, mild exercise is important when you suffer from hip arthritis. Most of our patients experience that the hip gets more flexible when the pain disappears after implantation of gold beads around the hip joint.

I am SO happy that my pain is gone

Now, almost three months have passed since I got gold in my left knee on the 3nd of November.
I’m happy to tell you that the pain was pretty much gone immediately after my gold bead implantation. The pain briefly returned at the end of November but then the pain disappeared again.
I still get some swelling of my knee (less than before) when I walk far. But my knee doesn’t hurt anymore and the swelling goes away fairly quickly. I can fully stretch my knee, which I couldn’t do before.

I am SO happy that the pain is gone and to have my full mobility back and it seems like it is still getting a little better day by day👏🏻😊.

To be considered for a our chronic pain treatment with gold implants, please do as follows:

  1. Contact the clinic to make an appointment (you’ll find our contact informations here). You can also contact us if you need to know more about the treatment before you decide to book an appointment.
  2. We need to see X-ray or CT- /MRI scanning images of the painful joint or painful area prior to your gold bead implantation. Your X-ray or CT-/MRI scanning should not be older than 2 years.
  3. You can always contact the department where you have had the examination. They can send you the images or description of the X-ray or CT- /MRI scanning The department is obliged to send you the images.
  4. Please send us the X-ray images in advance of your consultation.
  5. If you are taking anticoagulants, you should pause the medication some few days prior to your gold treatment. Always make sure to consult with us regarding any medications that you might take prior to your gold treatment. Please let us know if you take any kind of medication when you book your appointment.
  6. Please bring a relative or friend to transport you home after your gold bead implantation. We advise you not to driving your car for some few hours after your treatment due to the local anaesthetic.
